GENERAL INFORMATION

Age Requirements and Tuition:

          Movers and Shakers

                Infants (2-5 months)     Waddlers (12-16 months)

                   Babies (6-12 months)    Toddlers (16-20 months)

          Children must be two months up to twenty months

            by Sept. 1, 2008.

                             1 Day:  $80/month (T or Th)

                             2 Day: $110/month (T and Th)

          Early Learners

                   Younger Twos:

                   Children must be two years of age by Jan. 1, 2009.

                             1 Day:  $90/month (T or TH)

                             2 Day: $120/month (T and Th)

3 Day: $140/month (T, W, and Th)

                   Twos:

                   Children must be two years of age by Sept. 1, 2008.

                             2 Day: $120/month (T and Th)

3 Day: $140/month (T, W, and Th)

                   Threes:

                   Children must be three years of age by Sept. 1, 2008.

                             2 Day: $120/month (T and Th)

3 Day: $140/month (T, W, and Th)

5 Day: $170/month (M-F)

                   Fours:

                   Children must be four years of age by Sept. 1, 2008.

                             2 Day: $120/month (T, W, and/or Th)

3 Day: $140/month (T, W, and Th)

5 Day: $170/month (M-F)

Note:  In order to register your child for a 3- or 4-year old class, he/she must be COMPLETELY TOILET-TRAINED.

Hours:

The Early Learning Center is in session from 9:00 a.m. to

1:00 p.m.  Car pool drop-offs will not begin before 8:50 a.m. and will not continue after 1:10 p.m.

REGISTRATION

Registration begins Tuesday, February 19, 2008, for all ELC students.  Registration will open to the public on Monday, March 3, 2008.  The registration fee covers insurance and supplies and is completely NON-REFUNDABLE.  It also secures a place for your child in the program. 

Registration fee for one child:                                     $100.00

Registration fee for two children (same family):       $150.00

Maximum family registration fee:                              $200.00

TUITION

Tuition is due the FIRST DAY OF EACH MONTH in advance.  Tuition not paid by the 10^th day of the month will incur a $10 late fee.  Make all checks payable to SMBC EARLY LEARNING CENTER or ELC.  Please include child’s name in MEMO section of check.

HEALTH AND SAFETY

          1. An ALABAMA blue Certificate of Immunization prepared and signed by your physician’s office must be on file with the Program Director prior to your child beginning the Program.  It is required to be kept up-to-date AT ALL TIMES.

          2. A Notarized Emergency Release Form must be completed and returned prior to your child beginning the Program.

          3. A COMPLETED Registration Form must be returned prior to your child beginning the Program.

          4. A copy of documentation explaining/describing any emotional/physical challenge(s) your child may have.  This must be signed by your child’s attending physician.  This information will be kept private and confidential within your child’s file.
